4th Annual T-Shirt Design Contest - 2001

T-shirt Design Contest Rules

1. Artwork must be accompanied by a copy of this permission form, which may be obtained from the School Art Teacher, by contacting MADD at (714) 838-6199 or by printing this form from the website.
2. Students must use this year's theme: "There are faces behind the statistics."  Only designs with a "no use" message will be considered for First Place, as many of these T-shirts will be distributed to those under the age of 2 1.
3. Entries must be designed for one side only, and be no larger than 11" x 11".  The designs must be no more than 4 colors (black and white each count as one color). 
4. Artwork must be submitted along with the entry/permission form to the MADD, Orange County office and postmarked no later than Friday, March 9, 2001. 
5. All artwork must be original and the student's own creation.  Artwork must not be fashioned after or appear similar to trademarked or copyrighted material. 

T-shirt Design Contest Permission Form

The undersigned hereby grants permission to Mothers Against Drunk Driving, Orange County Chapter (referred to as "MADD"), to use the following artwork in MADD's T-shirt Design Contest. MADD retains the right to make any necessary alterations to the said artwork (in color, size, etc.) for reprint. Additionally, MADD is granted permission to utilize your artwork in other mediums. 

It is understood that granting permission to MADD in no way restricts re-publication of the material by the undersigned or others authorized by the undersigned.  The artist retains all rights. 

One copy of the permission form shall be returned to MADD and one copy shall be retained by the undersigned. Please print legibly or type.
